Thank you for your interest in enrolling your child at the Learning Spot Child Development Center! Below you will find a Contact Information form we welcome you to complete. We would love to take a few minutes to discuss what we can offer your child's future. At the bottom of this page you will find links to forms which you can complete prior to your meeting at our facility, or, if you wish, we can provide you with the forms when we happily greet you for a personal tour of our school!

Please fill out your information below to be contacted by one of staff regarding enrollment of your child at our facility. Please be sure to include a phone number and/or e-mail address so that we may contact you. You may also use this form to request an invoice for electronic or credit card payment via PayPal. If you are requesting an invoice, please be sure to include your child's full name in the comment box. You may submit one request for multiple children. If you are requesting an invoice you MUST include your email address in the request. Thank you! Be advised that there is a $2.50 processing fee PER CHILD for all credit card transactions.

ENROLLMENT FORMS

If you are interested in enrolling your child, below you will find forms which are required to be completed prior to your child's attendance. You may either print these forms and begin completing them in the comfort of your home, or, if you prefer, we have them available in our office and they can be completed in person. It is not mandatory nor required to have the forms completed before touring the school or making an appointment for enrollment.
